Diversified Home Health Care, Inc., (DHHC) is a Medicare certified home health care company servicing Southeastern, Michigan since 2001. As a dependable home care provider for more than 10 years, we provide skilled and non-skilled medical services for adult, seniors and pediatric patients in the comfort of their homes, Assisted Living Facilities, Nursing Homes, Group Homes and Adult Foster Care setting. We provide Companions, Certified Nurses Aides, Registered Nurses, Care Managers, Registered Nurse Care Managers, Home Makers, Skilled Nurses and Therapy services.

Our goal at Diversified Home Health Care, Inc. is to restore and maintain the client’s optimal physical, emotional, and social functioning. DHHC, Inc. help our patients reach a level of well being that work best with personal lifestyles and environment. We involve our network of trained personnel, client’s physicians, family, and community resources.

We provide short term relief in the form of respite care or a more permanent care arrangement for a loved one or yourself, our caregivers can help make life easier. DHHC’s staff is carefully screened and credentialed, and has experience providing care for adult, geriatric, and pediatric patients with a variety of medical needs.

Although most home care requests are usually planned, and families need the time to research and evaluate many solutions Diversified Home Health Care, is able to respond quickly to those families and clients that require a faster response in situations as a result of an emergency or a late day discharge from a medical facility such as a hospital or rehabilitation facility. We can respond to requests for in hospital assistance, assisted living facility bedside care or to someone who is being discharged from the hospital or rehabilitation.  We are also available to assist with major transitions such as assisted living to a family member’s home, or into a family member’s home as part of a relocation.

OFFERED SERVICES

DHHC provides Registered Nursing and Licensed Practical Nursing that supports the level of care needed to meet the increasing medical needs of a loved one. Our goal is to allow patients to stay safely at home rather than in a nursing facility or hospital. Our services included wound care, diabetic care, catheter care, cardiac and respiratory assessment and, medication management, pain management, and cancer care.
Occupational therapy assists patients with cognition, using memory building techniques and appropriate executive functions, such as handling personal finances. This program helps patients take care of themselves when a health problem limits normal, day-to-day activities, such as bathing, dressing, eating, preparing meals, housework, and managing personal business. Our therapy can help patients become more active and willing to live independently.
Our medical social workers implement treatment programs, referrals and coordination of community resources and programs to assist home-bound clients and their families to adjust to personal and environmental difficulties which predispose illness or interfere with obtaining maximum benefits from medical care.
Our physical therapists work with you to improve and restore mobility and muscle strength. Skills are taught in the comfort of your home by specially trained physical therapists.

Our licensed physical therapists are experienced in helping with recovery from issues such as fall injuries, joint replacement surgeries and stroke-related disabilities. Your individual treatment plan is designed based on your needs, with the goal of achieving the highest extent of recovery possible. Our physical therapy services help you learn how to restore function through education on balance and coordination activities, therapeutic exercises, home safety evaluation, fall prevention, proper body mechanics and specifically designed exercises.
Speech therapy helps patients improve their ability to communicate. This includes speech, which is how sounds are made, and language, which involves understanding and choosing the correct words. This can include working with patients who have learning disabilities and hearing loss. A speech therapist can also help patients with swallowing, drinking, or eating difficulties, as well as helping patients when their dietary needs are affected by memory loss or dementia.
For many seniors or disabled individuals living at home is a priority. DHHC provides in-home services that can help prolong a person's ability to live safe and independently. Our care lends a helping hand while enhancing the quality of our clients home environment. We provide a variety of in-home services for those in need including: Housekeeping, Meal Preparation, Grocery Shopping, Errands, Laundry, Grooming Guidance, Bathing, Hygiene, Mobility assistance, Transferring, Toileting and Incontinence Care, Feeding, and Dressing Assistance.
